The video above provides step-by-step instructions on how to enroll your employer’s 401(k) plan utilizing Slavic401k.com.
1. Go to Slavic401k.com and click “Enroll Now / Opt-out” on the home page.
2. Enter your social security number and date of birth. This allows us to pull up your records.
3. Click “Start Enrollment”
4. Your name and other personal information will be pre-populated based on records given to us by your employer. If necessary, you can update or change your contact details before moving on. Check the box to confirm that you have received the disclosures and then click “Save & Next”.
5. Determine how much of your paycheck to contribute. If you are unsure, we have calculators here to help.
6. Select an investment option. The options available to you may be different than what you see in the video above. Select how you would like to invest in your 401(k) and follow the prompts to make fund selections.
7. Once your investments are selected, add beneficiaries to in case you’re unable to receive your funds. Enter your full name, date of birth, social security number, relationship, and percentage. Be sure the beneficiary recipient percentage allocation adds up to 100%.
If you are married and wish to name someone other than your spouse as the primary beneficiary, your spouse must consent to the designation by completing the Spousal Consent Form.
8. Review account information and modify if necessary. Click “Confirm”.
9. Now simply read and sign the authorization form to complete the process. Elect if you would like to opt-in for electronic delivery and email express. Select checkbox to confirm that all information provided in accurate and complete and click “Submit Application”.
You are now enrolled in your 401(k)!
